Refer a Child
Sunshine fulfills dreams for children all across Canada, all year long. To make a dream come true for a child, the first step is submitting a request for review.
You can refer a child by completing and submitting a Referral Form (PDF). The form seeks only basic information about the child: name, diagnosis, age, and home contact information. When Sunshine receives this form, our staff will contact you and send a full Individual Dream Application to the child’s family.
The Individual Dream Application gathers more detailed information: about the child; his or her diagnosis; and his or her dream. The child’s physician must also complete part of this application.
Following approval, Sunshine works as quickly as possible to make the dream come true.
